Selecting Agents

When you are considering naming an “agent” you are appointing someone to a position of trust to make decisions, financial and medical, for you in the event you are unable to make those decisions yourself.  We are talking about you being incapacitated to the point you cannot make decisions regarding your own (or anyone else’s) medical or financial needs.

The typical documents requiring an agent are:

  1. Financial Power of Attorney (known as a Statutory Durable Power of Attorney in Texas)
  2. Medical Power of Attorney (sometimes referred to as a Health Care Proxy or Power of Attorney)
  3. HIPAA Release and Authorization

We apologize but at this time the Secure Wills™ website does not provide templates for those documents although work is in progress!  The anticipated release date for those forms is in January of 2010, so please check back with us and get those forms in place.  If you like, forward your e-mail address to customerservice@securewills.com and we will contact you when the new forms are available.  You do need them in your plan alongside your last will and testament form you are creating today.
